The Degenerative Spine: Clinical Diagnosis and Management for the Chiropractor

Degenerative changes of the cervical and lumbar spine are among the most common findings encountered in chiropractic practice. However, radiographic degeneration does not necessarily identify the patient's pain generator or determine the appropriate treatment.

This 2-hour continuing education course provides practicing chiropractors with a clinically focused approach to evaluating and managing patients with degenerative spinal conditions. Emphasis is placed on differentiating mechanical axial pain, radiculopathy, spinal stenosis with neurogenic claudication, degenerative spondylolisthesis, and degenerative cervical myelopathy.

Participants will review focused orthopedic and neurological examination, red-flag recognition, appropriate utilization and interpretation of diagnostic imaging, patient selection for spinal manipulation and mobilization, exercise prescription, progressive loading, outcome assessment, documentation, and appropriate referral or co-management.

The course emphasizes clinical correlation rather than treatment based solely upon degenerative imaging findings, and integrates conservative chiropractic management with active rehabilitation and objective reassessment.

Learning Objectives

Upon completion of this course, the participating chiropractor should be able to:

1. Differentiate common degenerative imaging findings from clinically meaningful mechanical, radicular, stenotic, and myelopathic presentations.

2. Perform and interpret a focused orthopedic and neurological examination, including root-level motor, sensory, reflex, gait, and upper-motor-neuron assessment.

3. Recognize clinical red flags including cauda equina syndrome, progressive motor deficit, degenerative cervical myelopathy, fracture, infection, and malignancy.

4. Determine when radiographs, MRI, CT, specialist referral, or medical co-management are appropriate.

5. Select and modify spinal manipulation, mobilization, exercise, and progressive rehabilitation according to the patient's diagnosis, neurologic status, bone health, symptom irritability, functional limitations, and response to treatment.

6. Apply objective outcome measures and reassessment findings to determine whether care should be progressed, modified, discontinued, or referred.
